How to use these
forms:
These forms are available online in Adobe Acrobat format. They
require the Acrobat Reader v3 or higher to be installed on your
computer.

Depending on the form and the speed of your Internet connection,
there will be a delay of one to five minutes while the selected file
transfers to your computer. Use the PRINT controls in the
Acrobat Reader (not the browser) to send the form to your printer.
Once the form has printed, fill it out normally, then submit it to
the appropriate department. Please be sure to print and fill
out each page of multi-page forms before submission.
These
forms were scanned and converted from our standard printed forms.
There will be some blurriness and misalignment in your printouts,
but the result should be acceptable. If you have difficulties,
please consult your printer/software documentation, or contact your
IT advisor.
